The request for funding must be to meet gaps in public funding for medical, dental, and/or ocular needs andsupporting services, and/or for other activities, goods, and/or services that will enhance the quality of life of the applicant.

Applications for financial support from the Gift of Life America Fund, Inc. may be made at any time by the parent(s), legal guardian(s), or caregiver(s) of the children. In addition to completing an application form and providing all specified information, the applicant(s) should be willing to be interviewed by the Fund Committee.
